FILE OF LIFE

Dear Residents,

    The Jordan Volunteer Fire Department and Ambulance is providing free for the residents of the Jordan, Elbridge and Memphis area the File of Life.  The File of Life is an information card containing important medical information that magnetically sticks to your refrigerator.  The information is supplied by you or your physician and updated as needed.  This enables the ambulance personnel to assist in your care, especially at times when you are unable to.  Please take the time to fill this out in pencil, so changes can be made if necessary.  There should be a File of Life for each person in the household.  During times of medical emergency it is necessary to have accurate and current information, so keep this updated.  Let your primary care physician know you are using the File of Life and they should be happy to assist you.  If you know of someone in need of a File of Life, or need a new one, please let us know.  Also, please call us with any questions; we are always there to assist you (689-3923). If we are out on an emergency and no one answers, please leave a message and we’ll get back to you.
